How To Make an Appointment

Most patients are referred to physicians at the Eye Institute by their personal doctor. Patients who wish to see an ophthalmologist at the Eye Institute can discuss this with their personal doctor or call the patient scheduling coordinator at one of the phone numbers listed below. Managed care patients will need a valid, signed referral from their managed care plan (primary care physician).

To find out which managed care providers are accepted by the Eye Institute, please see our listing of approved Managed Care contracts and insurance providers.
